I, {name}, confirm that I am physically fit, that I have discussed studying Judo and Jiu-Jitsu with my physician and that there are no reasonable grounds for concerns about my health or the health of other students that arise from my participation in Judo and Jiu-Jitsu. I promise to immediately advise the Kohbukan-Sisu Judo & Jiu-Jitsu Club (the "Club") if I become aware of any such concern.
I recognize that regular class attendance is important for my progress and understand that it is my responsibility to attend the classes. It is further understood that I am obligated to make payments whether or not I attend class. I further understand that failure to complete the lessons does not relieve me of my obligation to pay the tuition in full.
I understand that during the course of instruction, the Club's instructors and/or other students or authorized persons will be engaged in a course of conduct requiring physical contact; and I give full consent to such contact as required by training.
I understand that the Club will not be held liable for any injuries, damages, etc. not caused by or resulting from negligence of the owners, operators, or persons in charge of such establishment, or their agents, servants or employees.I acknowledge receiving a copy of this agreement.
I promise to abide by and accept all the rules of Judo, Jiu-Jitsu and of the Club and to comply immediately with any direction given by an instructor. I also promise to advise an instructor if I observe behaviour that does not comply with the rules or is otherwise potentially dangerous to students or anyone else.
I agree to register for Judo Ontario membership if myself or my child is participating in Judo and Ontario Jiu-Jitsu Association membership if myself or my child is participating in Jiu-Jitsu.
I agree that any photos or videos taken of me or my child while I am at the Club will belong to the club and can be used on social media or for advertising purposes.
Notice of Consumer's Rights
You have the right to cancel within five (5) working business days after the receipt of a copy of this contract. Cancellation must be in writing and delivered in person or by certified or registered mail.
If you become disabled for at least (6) months during the membership terms and that disability is confirmed in writing by a physician, you have a right to an extension of this contract.
If the school is closed for a month or more, you are entitled to your choice of either extension of the contract or prorated refund, except if the closing is not the fault of the facility, in which case the choice of remedy is the school's.
You may also cancel if you move your residence more than 25 miles from the place of instruction. Cancellation under this section requires written proof of new permanent address, phone number, name and address of new employer (if have one) and requires 60 day advance written notice. A residence investigation will then be conducted.
